I only know

...there is one at NYU. I'd actually be surprised if there isn't one near St. Louis. But your neurologist should know what's there, and if you haven't been referred yet, they may not be on board.
I have to say that supraorbital n. decompression is news to me, though I'm hardly an expert. Nerve decompression is well-established for trigeminal neuralgia, where they actually move a blood vessel away from the nerve trunk.
Is it a muscle that they cut to decompress the nerve? Certainly the fact that botox worked sounds like a pretty good sign that decompression might work. But I have to say--the fact that the surgery has only been adopted by plastic surgeons (and not neurosurgeons) gives me pause
